I think the guitar you are looking for is from the brand Reverend, maybe the slingshot model but I´m not sure. On amps he used mesa boogie double rectifier but now he is endorser of the brand bulldog.

I believe the guitar is a Reverand Rocco, which is a discontinued model no longer made. I think the Rocco and Slingshot are basically the same, just with different pickups (Slingshot has soapbars, the Rocco humbuckers). Reverend Rocco - Lime Green
